Sick certificates (Med 3 Certificates)

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

You can complete the ‘Employee’s statement of sickness’ form online.

If you attend hospital or have a procedure or operation in hospital the hospital consultant or Doctor can supply you with a Med 3 Certificate.

Med 3 certificates are required for Social Security and Statutory Pay purposes for patients who are either incapable of work or who may be fit for work with support from their employer. The duty to provide a MED3 rests with the doctor who, at the time, has clinical responsibility for the patient so if a Hospital Consultant or Hospital Doctor tells you that you should not return to work it is their responsibility to supply you with a Med 3 (formerly known as a sick note).

Often the Med 3 certificate can be emailed to you if we have a verified email address on your record.

If people need a sick note for non-covid related illness after 7 days of self certification they should contact the GP.

Silton Surgery cannot supply patients with an exemption certificate regarding the wearing of a face covering.
