DURING COVID19 OUR USUAL APPOINTMENT SERVICE IS SUSPENDED. PLEASE RING RECEPTION ON 01747 840226 TO ASK WHAT SERVICES WE ARE CURRENTLY SUPPLYING AND HOW TO ACCESS THEM. IF YOU NEED TO BE SEEN YOU WILL BE, BUT IN THE FIRST INSTANCE YOUR NEEDS WILL BE ASSESSSED OVER THE TELEPHONE. THIS IS TO PROTECT ALL OF OUR PATIENTS AND STAFF. WE ENCOURAGE PATIENTS TO CONTACT US WITH ANY CONCERNS THEY HAVE ABOUT THEIR HEALTH WE CAN THEN TAKE STEPS TO MANAGE ANY RISKS AND HELP THE PATIENT TO OBTAIN WHAT THEY REQUIRE.

Please complete and return a travel questionnaire, either download from here: TRAVEL_QUESTIONNAIRE or obtain a hard copy from the surgery. (The travel questionnaire should ideally to be completed and returned by the traveller/s prior to appointment)

Available for all patients aged over 40 with the Practice Nurse. IF YOU RECEIVE AN INVITATION FOR A NHS HEALTH CHECK PLEASE BOOK AN APPOINTMENT

It is recommended that everyone over the age of 75 has an annual health check up. Please make an appointment with the Practice Nurse if you would like this. At this appointment, we may ask for details of your next-of-kin or a friend that we can contact should there be an emergency.
It is also suggested that everyone between the ages of 16 and 74 has a check up every three years. Appointments for this should also be made with the Practice Nurse
