If you are not able to complete and return one of our travel questionnaires at least 8 weeks before your date of travel we will not be able to deal with your travel needs. You will have to attend a private travel clinic.
Before travelling abroad you need to download the ‘Travel Questionnaire’ below, (or collect a hard copy from the surgery).
Please complete and return the questionnaire to the surgery, at least 8 weeks before traveling, in order for the nurses to check your record and details of your travel plans. If vaccinations are required we will contact you to book a consultation.
Our Practice Nurses, Jessica and Claire can give travel advice and administer travel vaccinations. Malarial Treatment and some of the travel vaccinations are private, so therefore chargeable. You can obtain details of the cost of travel vaccinations and malarial medication from the Dispensary. We do not administer the Yellow Fever Vaccination.
You can also access travel vaccinations from a private travel clinic who will charge for all vaccinations.
