Travel Advice: Practice Nurse You can download a Travel Questionnaire from the website: TRAVEL_QUESTIONNAIRE or pick up a copy from the surgery. Please return the completed questionnaire to the Surgery as soon as possible. (If you require a travel consultation this should ideally be 8 weeks prior to travel). Malarial treatment and some holiday vaccinations are private so therefore are chargeable. WE DO NOT ADMINISTER THE YELLOW FEVER VACCINATION AT SILTON SURGERY.
Weight Management: Practice Nurse
Health checks: Available for all patients aged over 40 with the Practice Nurse
It is recommended that everyone over the age of 75 has an annual health check up. Please make an appointment with the Practice Nurse if you would like this. At this appointment, we may ask for details of your next-of-kin or a friend that we can contact should there be an emergency.
It is also suggested that everyone between the ages of 16 and 74 has a check up every three years. Appointments for this should also be made with the Practice Nurse
